VA Doc won’t take me serious

So after not seeing the VA for over a year because I live 2+ hours from a VA clinic, I finally went. Took a day off of work and made the 4 hour round trip. Brought up a couple conditions bothering me because why not haven’t seen the doc in over a year and I don’t wanna have to come back for awhile. Condition one, I told him I have pain in lower back with burning, tingling, numbness down my left leg and foot. Checked my healthevet today and all he wrote on there was intermittent back pain WITH NO pain down the legs. I was like come on dude I specifically told you I had pain in my legs. Second condition I told him I’m becoming more and more forgetful. I was honestly scared I forget things that happen just a day or two ago. I’m only 32. He brushed me off and said it’s just anxiety and I need to relax. He didn’t even list my forgetfulness/ burning and tingling on myhealthevet doctor notes nor made any referrals for me to get treatment. Just gave me muscle relaxers and sent me on my way. Any way for him to at least document my concerns that I went in to his office for? I want the things that bother me to be at least documented and not left out as well as getting treatment!
