Has anyone been denied service connection for a documented condition due to a PHA answer?

I’m denied service connection for my back with several encounters in AHLTA due to a PHA I had marked I wasn’t having back pain at the time.

I have gotten documentation from the VA showing my back is now messed up from a surgery I had on AD causing leg length discrepancy.

I will be using that to attempt to show service connection, but other than that, has anyone had success overturning a NSC decision for a PHA answer?
