I’m not sure how to proceed forward - can someone help?

I’m rated for the following:
70% PTSD, major depressive disorder and alcohol use disorder, moderate, in early remission
40% Fibromyalgia
30% Myofascial Pain disorder with bruxism
10% left knee strain
10% right knee strain
10% tinnitus
I currently have a claim in for migraines secondary to bruxism. This claim has been in for over a year and I keep getting papers in the mail saying they will call me apologizing for the delay. Should I call them? These migraines make me call off of work often. I’ve missed on average 3-4 work shift mornings a week. The only reason I still have my job is because my boss vouches for me and I just get my work done in the evening.
Additionally, there’s some other things I don’t have diagnoses for that I believe are related to my military service.
One is that my stomach hurts anytime it’s pressed lightly. Even when I lay down on my stomach. I’ve asked multiple doctors to look into it and they all say it’s constipation - then give me laxatives which don’t take the pain away. This all started when I was at a training facility in 2016. I’m sorry if this is a stupid question. But can I put in a claim without a diagnosis?
Another issue is I’ve had a hemorrhoid since being in too that still hasn’t gone away. Someone told me I can claim this, is that true?
Finally if I am not regularly going to the doctor to get treated for my ailments that I have claimed, can they take away my disability? The VA is about a 50 minute drive from me and I’m not able to do that drive often.
