Is an NCO allowed to force someone to answer a personal questionnaire?

So my wife’s NCO and her do not have a good relationship. They are both professional at work in uniform, but my wife doesn’t share any personal information with her because of a prior incident where her NCO called her a piece of shit soldier for getting pregnant. My question is, my wife has been under this nco for a year now, and she recently gave her an “initial counseling” form that is just a personal questionnaire. She told her it’s mandatory, but does she actually have to answer it because my wife just wants to avoid sharing personal stuff and not create more points of friction. Some of the questions are “What’s your families background” “tell me about your kids” and “how do you define spiritual success”. It’s clear the nco just typed this up on a word document so I don’t want to just post it, and it’s not on a 4856, so this nco will know if i post it.
