ACFT 3.0 scorecard to use for dependants, to grade 5, 12, and up to 16.

I just want to make sure my family is properly aware of their own fitness, so that can seek me out if they need assistance or can't find it themselves (I've also already taught them how to appropriately research things they are unfamiliar with, so I should be the last resource they use).
As a result of this decision, I've informed them that the entire family will be conducting a diagnostic ACFT on Friday. A few other Soldiers from my unit thought this was a fantastic idea, and very eagerly volunteered to assist with setup, teardown, and grading. I signed out two sets of weights and everything this morning, and they're sitting in the back of my Mazda right now.
The only thing I'm missing, however is an appropriately scaled scorecard that starts at 5 years and goes up to... what, 60?
I'm sure someone has done this for likely similar reasons before, or has the ability to quickly figure out the age and score algorithm, so that it could be made higher, just as much as it could lower.
I told them that if they failed the record in a month, they'd lose access to their preferred electronic device for the next month or until they passed. After all, what are you supposed to do with a 5 or 13 years who can't overhead yeet?
