unfit for duty, mental disability

Ill make this story short.
There was a guy in my base who, through no fault of his own, wasnt very smart.He was so clumsy and frankly idiotic he should have been medically discharged for intellectual disability.
So there are two stories about him that go hand to hand.
On a shooting practice, we had two signals to do with our feet from "prone" shooting position.
left foot up: magazine empty, waiting for further orders
right foot up: any sort of issue, calling for the officer in charge of the line
now right foot up could mean everything, including weapon malfunction with a live round in the chamber and the trigger pulled
this guy has a live 7.62 round in the chamber with a weapon not firing after the trigger was pressed. Instead of raising his right foot and waiting for assistance while holding the gun in case it was a delayed action, he stands up, with the weapon turning it towards his C.O at a range of 5 meters. C.O drops to the ground hands over his head, screaming to turn the weapon towards the targets and hold it there.
Unrelated incident:
We have an SOP of emptying our weapons in the air (removing the clip, arming the weapon and firing in the air). We do this about 3 to 4 times after any firing exercise and every day. This guy 2 whole days after our live fire shooting does the procedure and what do you know a bullet drops on the ground
